1.  Be sure you know the name of your group (your facilitator should have given this to you).  You must enter the name of your group exactly as it was given to you.

2.  Click on the link below to complete your self assessment.  During the assessment, you will receive instructions to pass on to your direct reports so that they can also rate you anonymously (do NOT direct them to the link below):
